Sole Proprietorship
Documentation Checklist

- IRS Employer Identification
- Number (EIN) Letter (if applicable)
- Certified Fictitious Business Name Statement (if applicable): Required if the owner’s surname is not in the name of the business or the business name implies the existence of other owners.
- Current Government Issued Photo ID & a second form of ID for each signer
Authorizing Individuals
The following authorizing representatives must sign to authorize the opening of the membership or authorize changes of signers:
- One Sole Proprietor
Partnership
Documentation Checklist
Download a printable copy of the business documentation checklist for partnerships, or review the items needed below:
- IRS Employer Identification Number (EIN) Letter
- Partnership Agreement
- Certified Fictitious Business Name Statement (if applicable).

- Required if Partnership is conducting business under a name that does not include the last names of all owners or implies the presence of additional owners.
- Required if Partnership is conducting business under a name other than what is listed in the Partnership Agreement.

- Current Government Issued Photo ID for each signer a second form of ID for each signer
- Certification of Trust (if applicable, can be completed at SESLOC)

- Required for any Trust that is a general partner
- Required for any Trust that is a limited partner and has a 20% or more ownership
- Must be dated within the last 5 years

- Certificate of Limited Liability Partnership (Form LLP-1) (Limited Liability Partnerships Only)

Corporation
Documentation Checklist

- IRS Employer Identification Number (EIN) Letter
- Articles of Incorporation
- Filed Statement of Information (Form SI-550)
- Meeting Minutes (member may use SESLOC form as a supplemental form to the organization meeting minutes). Officers must match the Statement of Information, and the Minutes must include:

- The name of the group on the letterhead
- The first and last name of each officer of the group
- Signature of the secretary of the group
- Requested actions to be taken on the membership (EX: Add or remove signer)

- Certified Fictitious Business Name Statement (if applicable). This is required if corporation is conducting business under a name other than what
is listed in the Articles of Incorporation. - Current Government Issued Photo ID for each signer a second form of ID for each signer
- Certification of Trust (if applicable, can be completed at SESLOC)

- Required for any trust with 20% or greater ownership of the corporation.
- Must be dated within the last 5 years.

Limited Liability Corporation (LLC)
Documentation Checklist

- IRS Employer Identification Number (EIN) Letter (optional for Single Member LLC)
- Articles of Incorporation (LLC-1)
- Filed Statement of Information (LLC-12): Manager(s) or Member(s) listed must match how the LLC is structured to be managed
according to the articles of organization - Operating Agreement
- Certified Fictitious Business Name Statement (if applicable). This is required if business is conducted under a name other than what
is listed in the Articles of Incorporation. - Current Government Issued Photo ID for each signer a second form of ID for each signer
- Certification of Trust (if applicable, can be completed at SESLOC)

- Required for any trust with 20% or greater ownership of the LLC.
- Must be dated within the last 5 years.

- A Manager or Managing Member
- One other Manager or Member (N/A for Single Member LLCs)
Nonprofit Corporation
Documentation Checklist

- Filed Statement of Information (Form SI-100)
- Meeting Minutes (member may use SESLOC form as a supplemental form to the organization meeting minutes). Officers must match the Statement of Information, and the Minutes must include:
